Social Worker 

The Social Worker assists residents and their loved ones transition into Long Term Care supporting the individual holistically. The Social Worker facilitates meetings and discussions to identify the resident’s goals of care throughout their stay at Rosewood Manor. Furthermore, the social worker supports resident’s personal needs by assisting with referrals to financial management agencies, and other community resources to meet the resident’s needs.
